  • The Utah Advance Health Care Directive (establishing Durable Power of Attorney and a Living Will)
  • Utah’s "Physician Order for Life Sustaining Treatment" (POLST), also known as the "Life With Dignity Order"

  • Tips for starting the conversation with family members
